At your request, we will submit billing directly to your insurance company. We ask that you provide us with all necessary information to accomplish this. Should you have a secondary insurance, we will submit to the primary carrier and after your primary carrier processes the claim we will submit the balance to your secondary carrier.

We are pleased to bill your insurance for you, however, we do require that you pay your CO-PAYMENT at the time of your visit.
